Contouring can also involve fat injection, fat removal, implants, and other techniques to provide volume and fullness to the aging face that has been lost over time.
For certain problems, such as excess submental fat (under the chin), facial liposuction, or lipoplasty, is a great option for patients This can be done at the same time as a facelift or as a separate procedure, when indicated. Liposuction techniques are also effective for sharpening of the jawline and reduction of joweling. This is useful for patients that are not ready for a facelift but want contour corrections along the jawline. The healing time after liposuction is also significantly less than with a facelift. A weak chin and prominent fat deposits in the neck and jowls can detract from the overall facial balance and harmony. Dr. Westreich performs liposuction to remove unwanted fat from these areas, with or without the placement of a chin implant for improved contour. This procedure is often combined with rhinoplasty, and blepharoplasty. Lipoplasty can produce a dramatically more attractive profile and more youthful facial shape.
Volume loss is a significant part of the aging process. This is especially true in the midface and lower eyelid region. Fat grafting is a 2 staged technique for replacing this lost volume using a patient’s own tissues. The fat is typically harvested from the lower body (abdomen, thighs or buttocks and then processed for implantation into specific areas of the face. Typically, this procedure is done under anesthesia, which is what makes it different from commercial filler products, such as Restylane or Juvederm. Fat grafting, also known as fat transfer, can also provide permanent contour corrections. Although some fat will survive permanently, you will need a touch up in one or more years to maintain a lasting result. Fat grafting is often done at the same time as facial liposuction or eyelid surgery.
Dr. Westreich specializes in placing facial implants, such as chin and cheek implants from within the mouth, which avoids visible scars on the skin itself. The choice of implant and the specific indications for their use will be discussed with Dr. Westreich during a full cosmetic consultation.

Dr. Westreich approaches the cheek or malar area by using fillers such as Restylane and Juvederm, your own fat, as well as other implant materials when additional contour may be needed.

Although sometimes performed as a single procedure, chin augmentation is often performed with rhinoplasty, facelift, or submental liposuction procedures. Implantation is performed through either the mouth or a small external incision hidden below your chin. Implants are usually made out of silastic; an inert substance that is well tolerated by the body.
Bone remodeling (also called a sliding genioplasty) is a more extensive procedure than implantation. It can be helpful for selected individuals who have significantly smaller chins or poor lower lip competence. Sliding genioplasty is always done using incisions within the mouth and requires a longer healing period than implantation. Chin reduction, if needed, can be accomplished in a similar manner but is typically done with incisions below the chin.
